Our mission is to ensure comfort and empathetic care to
patients and their families who are facing a life-limiting illness. We are dedicated to helping them celebrate life’s

Hospice care is provided as a team approach. When you or your loved one enrolls in
hospice, the following team will work together with you to develop a plan of
care designed specifically for you and your needs. Other duties for these team members include:
- Medical Director
- Co-certifies
the terminal prognosis with the attending physician
- Leads
the interdisciplinary team in the development of a plan of care
- Provides
consultation to other physicians regarding hospice care
- Registered Nurse
- Assesses
patient and family needs
- Develops
plan of care to meet identified needs and coordinates team visits to
ensure implementation of approved plan of care
- Coordinates
care with attending physician, primary caregiver and/or long term care
facility staff
- Ensures
symptom control and pain management
- Provides
patient/family teaching as needed
- Social Worker
- Assesses
patient and family emotional, social, spiritual and financial needs
- Develops
a plan of care to meet identified needs
- Provides
direct counseling or refers patients and families to appropriate
community agencies
- Chaplain
- Assesses
patient and family spiritual needs
- Develops
a plan of care to meet identified needs
- Provides
direct counseling
- Assists
with memorial preparations
- Provides
consultations to community clergy
- Provides
support at time of death and during bereavement
- Home Health Aide
- Provides
direct personal care to patient
- Provides
comfort measures, reports identified needs to RN Case Manager
- Provides
emotional support to patient and family
- Volunteers
- Provides
companionship, support to patient and family
- Provides
needed non-medical services, letter writing, errands, respite time for
family
